Retirement is a major life transition. Work provides more than income—it gives us routine, social connections, belonging, identity, and purpose.
For older adults living with mental health disabilities, this transition can be far more challenging. Many already experience social isolation, limited support networks, and challenges with independent living. Some have been out of the workforce for years and rarely participate in social or community life.
As they age, isolation can deepen and essential skills, independence, and a sense of purpose may gradually decline.
Yet existing welfare and rehabilitation systems do not currently provide an adequate response for this population.
A Personal Path Back to Community
Chimes Israel’s Transitions Program provides gradual, personalized support based on each person’s abilities, interests, and pace.
We help participants take meaningful steps back into community life—from leaving home and joining social or leisure activities to building relationships and, when ready, finding meaningful volunteer opportunities.
The goal is not simply to fill time. It is to preserve life skills, strengthen independence and self-efficacy, and rebuild connection, belonging, and purpose.
From “I Need Help” to “I Have Something to Give”
Volunteerism is a powerful part of the program. For someone who has spent years receiving services and support, the opportunity to give to others can transform how they see themselves.
Someone is expecting them. Someone needs them. They have something valuable to give.
Transitions connects participants to existing opportunities in their communities, helping them become not only part of the community—but meaningful within it.

In 2027–2028, Chimes Israel aims to support 40 older adults living with mental health disabilities.

Because at every age, everyone deserves not only to be part of the community—but to be meaningful in it.
